Office Policies
We are proud you have selected CMP Pediatrics for your family’s healthcare. Over 2,000 valley residents choose our healthcare providers each day! In fact, 1 out of every 10 Central California residents have also chosen Community Medical Providers for their family’s healthcare needs.
The section below will help answer some FAQ’s about our office. If there is anything we can do to improve patient care, please don’t hesitate to contact us and let us know. Thank you for choosing CMP Pediatrics.
No-Show Fee
Beginning July 1, 2017, we will be implementing a $35.00 fee for:
-
Missed appointments (no show)
-
Failure to provide 24 hour notice to cancel or reschedule
This fee is your responsibility and is not billable to your insurance plan. If you have any questions, please ask!
Billing for Services
You are responsible for supplying all appropriate billing information, including insurance identification and the correct insurance billing address. Please notify us of any changes in your phone or address.
Insurance Plans
CMP accepts most health plans and we are a provider for most HMO plans in the area. Unfortunately, we are no longer contracted to see Medi-Cal recipients.
Appointments
If you need an appointment, please call us at (559) 437-7338. If your primary physician is unavailable, one of our other Pediatricians will care for your child. The findings and treatment during the exam will be communicated to your regular doctor.
Cancellations
We require a 24-hour advance notice if you need to cancel or reschedule an appointment. If you are more than 15 minutes late to your appointment, we may need to reschedule you.
The Basics
At times schedules may be affected by emergencies or longer-than–expected visits. We may need to address your urgent issues and schedule another appointment to address others. You will be taken into an exam room according to your scheduled time with your assigned provider, not your arrival time. We will do our best to keep you on schedule. If you need a second family member to be examined, please let us know when scheduling the appointment. Please bring to your appointment:
-
All medications you are taking
-
List of Urgent Issues
-
Forms: Physical, school entry, kindergarten, pre-school, camp, medication at school, WIC, daycare, sport physical, etc.
-
Insurance card (please bring to every visit)
-
Your Co-payment (amount is determined by your health plan)
-
Immunization records
When you need to have a prescription refilled, please contact your pharmacy at least 7 days before you run out of the medication. They will contact us if needed.
Mail Order Rx Refills
Please notify the office 15 days in advance for mail order prescriptions. For Controlled Substance re-ordering (ADD, ADHD, or pain medication), please call the front desk at 559-437-7338 and provide the following information:
-
Patient’s Name (please spell) & DOB
-
Daytime phone number
-
Name of medication
-
Strength of medication
-
Quantity of pills
-
How and when medication is taken
You will be notified by our office or your pharmacy when your prescription is ready. You will be informed whether you need to come to our office to pick it up or if the prescription was able to be submitted electronically.
Sample Medications
If you have received samples from your doctor, please follow the printed directions. You may fill any accompanying prescription if the samples were effective. All available samples will be given at the time of your visit.
Referrals
-
Routine referrals typically take between 5-10 business days
-
Urgent referrals – The decision for this is made by the doctor. The physician will advise our referral department staff on how urgent matter is and how soon patient needs to be seen
-
Basic appointment information: On the majority of referrals, the Specialist office will contact parents to arrange for the appointment. Direct communication works best between the Specialist and the parent.
After hours phone calls
-
We recognize that emergencies do occur after hours. If you have an urgent problem that truly cannot wait until the next office day, you may call 437-7338 or 437-7300 and the “on call doctor” will respond to you. They will try to get back to you within the hour. If you do not get a response within that time frame, please call again to make sure the message got through.
-
Antibiotics: We DO NOT call in antibiotics over the phone without seeing the patient. Our goal is to provide antibiotics only when absolutely necessary and to reduce the mounting resistance of bacteria in the community.
-
You may take children over the age of 2 to CMP Urgent Care. They are open as shown below and can be reached at 437-7304. Other Urgent Care facilities and local hospital information are also listed below.
